How do Community Health Workers help the community?

A Community Health Worker or a CHW assists the community by bridging gaps in the healthcare and social service systems. We are a care coordination agency offering case management to our members. If you are in need of services please connect with a CHW.

Does ACE offer Health Education?

ACE Community House currently offers a variety of trainings for business and community needs. Please consider us for Certified Grief Recovery Support, Infant & Maternal Health, Violence Prevention, Vicarious Traumas and Healing, Civic Engagement, Mentorship, CPR/First Aid, First Aid Mental Health, CQE/Re-entry, Social Justice.

I have been impacted by violence, I recently relocated or plan to relocate soon and want to apply for the Safe at Home Program, Can you assist me with this?

I am pregnant or recently gave birth. Can you help with resources?

ACE Community House partners with several organizations to provide resources. We can connect you to family advocates, lactation support, Doula, a parent coach and more. Ask us about free safe sleep assessments, cribs, car seats and linkage with A Step in the Right Direction free hygiene program.
